1. Project Description [10 points]
Describe the project to be funded and how it will improve library services and/or programming.

2. Project Relationship to Long Range Plan document [10 points]
Demonstrate how the project goals meet one or more of the six "LSTA Purposes"  as listed on page 7 of the Nebraska Library Commission's Long Range Planning document, LSTA Five-Year Plan 2008-2012.

3. Project Target Audience:  [10 points]
Describe the audience for the project and how the customer needs were determined. Explain how the project is designed to meet the need (s) identified.

4. Project Implementation:  [10 points]
Describe the plan to implement the project activities and/or services. Include a timeline that indicates a completion date.

5. Project Evaluation:  [10 points]
Describe the plan for evaluation of the project activities and/or services, including methodology for determining outcomes. Include details of how the information will be gathered and reported. (Note: Outcomes are related to the questions of whether or not your effort made a difference and if so, what that difference was. Outcome measures might include changes in attitude or behavior, documentation of knowledge, skills or abilities acquired, etc.)

6. Community Support: [10 points]
Provide evidence of commitments of financial and other support from community groups affected by this project, specifically related to the outcomes identified for this project.

7. Project Sustainability: [10 points]
Describe in detail how the program or services implemented in this project will be supported following expenditure of grant funds. Equipment acquired with grant funds must continue to be used for project purposes for at least five years, or the life of the equipment.

8. Project Communication Plan: [10 points]
Describe how library staff will communicate with the target audience regarding the program or services implemented in this project. Copies of communication pieces must be included in project reports.

9. Project Budget: [10 points total]
Briefly describe project costs and how the costs were determined. Complete the spreadsheet that shows budget details. Specify the amount requested in each appropriate category below.

Grants require at least a twenty-five percent (25%) local match, of which at least 10% must be a cash match. The minimum grant award is $500.

Print applications must be postmarked on January 7, 2011 or hand-delivered to the Nebraska Library Commission by 5:00 PM (CT) on the same date. Faxes are not accepted. The Library Commission will post a list of applications submitted.
