Application Information

 

Applications must be submitted in paper form and include the original signature of the certifying authority. FAX or electronic submissions can not be accepted.


Individuals Living in Nebraska  

Submit an Individual Application Form.

Example: Individuals living in their own homes, with family members, or independently in assisted living centers.

 


Nebraska Facilities
 

Submit a Facility Application Form.

Example: Individuals living in or receiving service through nursing homes, hospitals, assisted living centers, senior citizen centers. If individual residents prefer to receive service directly (in their own name), they need to submit an Individual Application Form.

 


Nebraska Schools
 

Submit a Facility Application Form and a Student Enrollment Form.

Also submit an Individual Application Form for each student new to talking books. Both these forms need to be submitted only once. A medical doctor or doctor of osteopathy must sign applications for students with reading disability.

Also at the beginning of each subsequent school year, submit an Annual School Enrollment Form to show which students will be receiving talking book service during that particular year.

 


Additional Applications
 

Nebraska Talking Book and Braille Service also offers a limited Collection of Descriptive Videos for loan. If interested in receiving these videos, anyone who qualifies for our service may fill out a video registration form in addition to the application for service above. You must complete both the application for service and the Descriptive Video Registration Form to receive videos.

After you have signed up for Talking Book Service, please inquire about our Braille and Audio Reading Download System (BARD). The BARD website has thousands of audio files of recorded books and magazine issues available for download. These audiobooks can be played using the NLS Digital Players we furnish our patrons.

Adobe The forms listed above are in PDF (portable document format). Acrobat Reader (version five and higher) provides text access to these forms. A user can enter information directly into the form using the computer then print the completed form or print the form and complete it with pen.
